Nurses lead the way in Post-Stroke recovery support

Summary
This study is observing whether a nurse-led support program helps stroke survivors and their caregivers after leaving the hospital. It follows 130 participants in Tanzania for six months to see if the program improves patients' confidence in managing their health, their quality of life, and how prepared they feel to go home. The goal is to understand if this type of ongoing care leads to better recovery and fewer hospital readmissions.
