Summary

This study tests whether special nurse-led education before hospital discharge helps patients better manage chronic conditions like heart disease and diabetes at home. Researchers will compare 120 hospitalized adults who receive this education to those who get usual care. The main goal is to see if patients become more confident and active in managing their own health over 3-6 months after leaving the hospital.
