Summary

This study tested whether a program combining nurse coaching calls and home monitoring of vital signs is practical and acceptable for older adults with heart failure. 46 patients participated after a hospital stay. Researchers measured how many people joined, stuck with the program, and found it helpful. The goal was to see if this approach could be tested in a larger trial to reduce hospital readmissions.
