Nurses and patients team up to tackle chronic disease

Summary
This study is testing a new way for nurses to work with patients who have long-term health conditions like diabetes, heart disease, or Parkinson's. The goal is to see if a partnership-focused approach helps patients feel more in control of their health and improves their quality of life. About 420 patients will work closely with an Advanced Practice Nurse to co-create a personal care plan.
