Summary

This study tests a 6-week nurse-led support program for family caregivers of people receiving home palliative care. The program includes education, phone coaching, and guidance on managing symptoms and medications. Researchers want to see if it improves caregivers' skills and reduces hospital visits. 150 caregivers will take part, with half receiving the program and half getting usual care.
