Can a 30-Minute nurse chat keep heart failure patients out of the hospital?

Summary
This study is testing whether a special education session led by a nurse can help people with heart failure stay healthier after they leave the hospital. About 118 patients will be randomly assigned to either get the 30-minute teaching session or receive standard follow-up phone calls. The main goal is to see if the education helps patients feel more stable and manage their condition better over three months.
