Summary
This small pilot study tested whether a support program could help older adults with heart failure manage their health at home. The program combined education before leaving the hospital, regular phone check-ins with a nurse, and sending daily health readings from home. Researchers wanted to see if this approach was practical to run and acceptable to patients, and if it might help reduce return trips to the hospital.
