Summary
This study is testing whether a personalized care plan can help prevent health crises for frail elderly people living at home. It involves 150 participants across 30 municipalities who receive home care services and are at high risk for needing emergency care. The goal is to see if this coordinated approach helps people achieve their personal health goals and stay safely in their homes longer.
