Summary
This study tested whether a case management program (the Navigator program) helps homeless adults get follow-up care with a primary care provider after being hospitalized. Over 650 homeless adults participated, and researchers tracked their outcomes for 180 days after discharge. The goal was to see if the program improves care connections and reduces hospital readmissions.
