Summary
This study looked at whether adding a clinical pharmacist to a mobile geriatric team could improve medication safety for hospitalized patients aged 75 and older. The team reviewed and optimized prescriptions for 200 patients taking five or more medications daily. The goal was to reduce potentially inappropriate prescriptions at discharge and three months later.
