Summary

This study is testing whether a mobile app can help people with heart failure take better care of themselves after leaving the hospital. The app provides educational content, symptom tracking, and reminders, and is used alongside standard medical care. Thirty adults with heart failure will use the app for 60 days, and researchers will measure how easy it is to use and whether it improves self-care and reduces hospital readmissions.
