Summary

This study is testing a new program designed to help doctors safely reduce or stop unnecessary medications for people 65 and older. The goal is to prevent harmful side effects and hospital visits caused by taking too many pills. It uses a computer tool to identify patients at highest risk and offers them a special clinic visit with a pharmacist to review all their medicines.
