NCT ID NCT06324045
Summary
This study is testing a new program where a team of healthcare professionals reviews and safely reduces unnecessary medications for people with chronic kidney disease. The goal is to see if this 'deprescribing' approach improves patients' quality of life, reduces hospital visits, and saves money for the healthcare system. Researchers will compare outcomes between patients who receive the program and those who receive usual care.
