Summary
This pilot study is testing a new way for doctors and pharmacists in Switzerland to work together to review the medications of older adults. The goal is to see if this team-based approach is practical and acceptable for improving medication safety. The study will involve at least 250 patients aged 65 and older who take five or more long-term medications. Researchers will check if this service helps reduce potentially harmful drug combinations and saves costs.
