This study looks at whether a special toolkit can help primary care practices get more older adults on Medicare to use their free Annual Wellness Visit. The toolkit includes electronic health record tools and practice redesign strategies. Over 22,000 patients across 24 practices will be followed for 24 months to see if the toolkit increases use of preventive services like cancer screenings and vaccines.
