Summary

This study tests whether a centralized care coordination system can help more people complete their recommended follow-up appointments after lung cancer screening. It involves over 6,700 current or former heavy smokers at community health centers. The program tracks patients, sends reminders, and provides support to overcome barriers to completing screening.
