New app and nurse team aims to keep dementia patients at home longer

Summary
This study tests whether a program combining specialized dementia nurses and a mobile app can help family caregivers keep their loved ones with dementia at home longer. About 554 caregivers and their family members will take part. The program focuses on reducing caregiver stress and managing challenging symptoms in the person with dementia.
