Summary

This study tests whether community health workers and special training for doctors can help more kidney patients complete the steps needed to get on a transplant waiting list. Five patients will be assigned to receive help from a community health worker who addresses social needs like housing or food, while their doctors get training to reduce bias. The goal is to see if this support speeds up the transplant evaluation process.
