Summary
This study tests whether a nurse-led remote monitoring program can help heart failure patients after they leave the hospital. About 200 patients will use a digital platform or phone to report symptoms and medication adherence for three months. The goal is to see if this reduces hospital readmissions and improves outcomes compared to usual care.
