Summary
This study tests whether personalized remote monitoring at home can reduce hospital readmissions for people with heart failure. About 200 patients who were recently hospitalized for heart failure will use technology to track their health from home. The goal is to see if this approach improves outcomes and helps patients manage their condition better.
