Summary
This study tests whether personalized nursing care and education can help adults with type 2 diabetes keep their blood sugar stable and improve their ability to manage the disease. About 72 participants will receive tailored interventions based on a structured nursing system. The goal is to reduce dangerous highs and lows in blood sugar and boost self-care skills.
