Summary
This study is creating and testing a new support program for Indigenous young adults in Maskwacis, Alberta, who are living with long-term health conditions. The program pairs them with a trained, community-based Indigenous Patient Navigator who helps them manage their health as they move from children's to adult healthcare services. Researchers will interview participants to see if this personalized support improves their experience and helps them feel more connected to the healthcare system.
