Community-Led program aims to ease healthcare transition for indigenous youth

Summary
This study is testing a new program where an Indigenous patient navigator helps young adults (ages 16-25) with chronic health conditions as they move from pediatric to adult healthcare. The program was co-designed with community Elders and members in Maskwacis, Alberta. Researchers will interview participants and hold talking circles to see if the support improves their healthcare experience.
