Summary
This study tests a program called THRIVE that helps people on Medicaid, including those with serious mental illness, transition from hospital to home care. Participants get a home care nurse within 48 hours of leaving the hospital and a doctor who oversees their care for 30 days. Researchers will compare readmission rates and emergency visits between those in the program and those getting usual care.
