New tool helps ER pharmacists catch medication mishaps in seniors

Summary
This study tests a simple questionnaire called AT HARM 10 to help pharmacists identify when a patient's hospital visit is caused by a medication problem. Researchers will enroll 100 adults over 65 in the emergency department to see how well the tool works. The goal is to improve treatment and reduce future hospital stays related to drug side effects.
