App and clinic team up to help ICU survivors heal and stay out of hospital

Summary

This study is testing a new support program for people who have survived a serious illness in the Intensive Care Unit (ICU). The program combines regular check-up visits at a special recovery clinic with remote support through a smartphone app. Researchers want to see if this integrated approach helps people recover their strength and thinking skills better, and prevents them from needing to return to the emergency room or be readmitted to the hospital in the first six months after going home.
