Summary
This study is for Veterans with advanced liver disease who want better care. It uses a nurse counselor over video or phone to help patients understand their illness, share what matters most to them, and align treatments—including possible liver transplant—with their goals. The study will track whether more Veterans get considered for transplant and have serious talks about their care preferences.
