Summary

This study tested a 12-week program to help African American and Latinx patients better manage their high blood pressure. A team including a community health worker, nurse, pharmacist, and social worker provided support and education. Patients used a cellular-connected blood pressure cuff for daily home monitoring while receiving weekly check-ins. The goal was to lower blood pressure and understand how social needs affect health outcomes.
