Summary
This study aims to see if a new 'hybrid' care model is practical for patients during major health emergencies like pandemics. It will involve 180 people with type 2 diabetes or heart failure who also have other chronic conditions. The model combines digital tools like remote monitoring and video calls with regular care, to see if it can reduce strain on the healthcare system while keeping patients safe.
