Study aims to ease Hospital-to-Home transition for sick kids

Summary
This study looks at ways to help parents of children with multiple health conditions or technology needs care for them after leaving the hospital. The goal is to reduce problems like readmissions or medication errors. About 282 parents will be surveyed about how well they understand and follow discharge instructions.
