Summary
This study is testing whether assigning a dedicated, consistent hospital team to patients who are frequently hospitalized improves their care. It will compare 1200 patients with complex medical needs who either get this special team or receive usual hospital care. The main goal is to see if patients with the dedicated team are more likely to see familiar doctors and nurses during future hospital stays.
