Summary
This study tests a new hospital support program designed to help patients with heart or lung disease quit smoking. A specialized team provides personalized medication plans, counseling, and follow-up text messages. Researchers will compare this approach to usual care to see if it leads to more people using quit-smoking medications and successfully staying smoke-free for six months.
