Summary
This study is testing a program designed to help hospital patients improve multiple health behaviors at once, like smoking, drinking, diet, and exercise. Nearly 800 patients will be randomly assigned to receive personalized feedback and motivation based on psychological theory or to receive standard care. The goal is to see if this proactive approach helps people make lasting, healthy changes to prevent common chronic diseases.
