Summary
This study is testing a new way to deliver palliative care at home for people with advanced heart failure or certain cancers. The program adjusts the level of support a patient receives based on how their symptoms and daily function change over time. It aims to improve quality of life, manage symptoms, and help patients discuss their future care wishes.
