Community health workers and home monitoring team up to stop high blood pressure

Summary
This study tests a program to help prevent serious high blood pressure. It combines home blood pressure monitors, a smartphone app for tracking, and regular support from community health workers. The goal is to see if this extra support helps people with slightly elevated blood pressure keep it under control better than just using a home monitor alone.
