Summary
This study aims to find the best ways for doctor's offices to use team-based home blood pressure monitoring. It involves 5,760 patients with high blood pressure who will regularly send their home readings to their care team. Researchers will test different support strategies to see which ones help patients control their blood pressure most effectively, especially focusing on reducing racial disparities in care.
