Cleveland clinic launches major push to tame high blood pressure

Summary
This study is testing a new team-based program to help patients with uncontrolled high blood pressure. The program expands support from pharmacists and other providers, using patients' home blood pressure readings to guide medication adjustments. It aims to help about 3,800 patients across 56 primary care clinics in Northeast Ohio achieve better, safer blood pressure levels.
