NCT ID NCT06908590

Summary

This study is testing whether a support program led by community health workers can help low-income heart patients access and complete cardiac rehabilitation after hospitalization. Researchers will measure whether participants find the program helpful and whether it increases their participation in recommended cardiac rehab sessions.
