Summary
This study is testing whether a support program led by community health workers can help low-income heart patients access and complete cardiac rehabilitation after hospitalization. The program involves 4-8 coaching sessions over 3 months to provide education, social support, and help navigating the healthcare system. Researchers will measure whether participants find the program helpful and whether it increases their participation in recommended cardiac rehab sessions.
