Hospital 'Tune-Up' aims to keep High-Risk hearts healthy

Summary
This study is testing whether a planned hospital stay for a full health check-up and personalized care plan can help lower the risk of heart attacks and strokes. It involves 70 adults who are at high risk for heart disease, either because they have multiple risk factors or have already had a heart event. The goal is to see if managing all of a patient's health problems together during this stay improves key health markers like blood pressure and cholesterol over the next year.
