Heart patients in need get tailored support to stay on track after rehab

Summary

This study looks for the best ways to help people from under-resourced groups keep up healthy behaviors after finishing cardiac rehabilitation. About 400 adults who have completed at least 6 rehab sessions will try different support programs, from low- to high-intensity. The goal is to find which approach works best for weight, physical activity, and medication adherence.
