Summary
This study aims to find the best way to help heart patients maintain healthy behaviors after they finish cardiac rehabilitation. Researchers will test whether simple text message reminders or a more detailed online program works better initially, and then test different levels of home-based support for those who need extra help. The study focuses on 400 adults from under-resourced communities who have recently completed cardiac rehab.
