Summary
This study is testing a new health management program designed for people with coronary heart disease who are also experiencing frailty (physical weakness). The program uses exercise plans and smartphone technology to help participants get stronger, walk farther, and improve their quality of life. Researchers hope this coordinated care approach will help control the disease and prevent it from getting worse.
