Summary

This study tested a coordinated care program for people with chronic heart failure, especially older adults recently hospitalized. The program combined daily at-home symptom and weight tracking via a tablet with nurse education and planned clinic visits. Researchers wanted to see if this integrated approach could help catch problems early and reduce the need for patients to return to the hospital.
