Weekly calls aim to ease the struggle of heart failure
NCT ID NCT07356843
Summary
This study is testing whether a support program called ADAPT can help people with heart failure feel better in their daily lives. The program involves weekly phone calls from nurses and social workers to help manage difficult symptoms and provide coping tools. Researchers want to see if this program can work well in regular community healthcare settings, not just in research studies.
