Summary
This study tests a new remote monitoring system called SmartSignalHF in over 1,100 adults with heart failure who already have a pacemaker-like device (ICD or CRT-D). The goal is to see if this smarter monitoring can reduce deaths and heart failure-related hospital stays over one year compared to standard remote care. Participants must be 18-85, have moderate heart failure (NYHA class II or III), and a weak heart pump (ejection fraction 40% or less).
