Summary

This study tests whether extra support from pharmacists can help patients with heart failure get the right medication doses when they have limited access to healthcare. Forty patients in Denver will either get these special pharmacist visits with easy-to-understand education or receive their usual care. Researchers want to see if this approach is practical and helps patients get closer to the recommended medication doses for managing their condition.
