Summary
This is a small pilot study to test a new smartphone app designed to help people with heart failure take better care of themselves after they leave the hospital. The app provides education, tracks symptoms, and connects patients with their care team. Researchers will see if the app is easy to use and if it helps improve self-care habits and reduce hospital readmissions.
