Summary
This study aimed to test a new support tool designed to help families care for children with serious heart disease after leaving the hospital. The tool was offered to a small group of families and healthcare providers to see if it was easy to use and helpful. The goal was to gather feedback to improve family confidence and communication during the transition from hospital to home care.
