Summary
This study is testing a program called HealthyLifetime, which uses nurse coaches to help older adults build healthy habits and self-care skills. The goal is to help participants maintain their health and ability to live independently in their own homes as they age. The program works with 120 participants via video calls to improve confidence, daily activities, and overall quality of life.
