Can a phone app help former inmates manage their health?

Summary
This study is testing whether a new personal health library app is helpful for people who have recently been released from jail or prison. Researchers will enroll 100 adults with a chronic health condition to see if they will use the app to organize their medical information and reminders. Participants will be randomly assigned to use the new app or a standard patient portal for comparison over three months.
