Personalized survivorship plans aim to bridge care gap for cancer patients

Summary
This study is testing whether a personalized survivorship care plan and a counseling session can help head and neck cancer survivors get better follow-up care. About 180 survivors will be randomly assigned to receive either this new plan or the usual care. The main goal is to see if the plan helps doctors implement more of the recommended follow-up actions over a year, which could improve patients' quality of life.
